Insurance and Rates
I accept the following insurance plans: HMSA, HMSA Quest, HMAA, HMA, UHA, UBH/UHC, and TRICARE. I also I have contracts with Employee Assistance Programs (EAP).
Co-payments or deductibles are collected at the beginning of each session in cash, check, or charge.
For "no call or no show" or cancellations with less than 24 hours notice, you will be charged a fee of $50.00 as this is not covered by insurance.
Please bring your medical insurance card and a photo I.D. to your first visit.
Please be advised that having insurance does not guarantee that your insurance company will make payment to me for the services you request. It is your responsibility to confirm what your coverage includes as well as your responsibility to pay for any amount that your insurance policy does not cover.
For out-of-pocket rates, please contact me directly.
